Ready for Intake

To request authorization for ABA services, please have the following ready for your call:

  • Full Diagnostic Report with an Autism diagnosis
  • Referral for ABA from your doctor or DPM (Diagnostic and Prescriptive Medical). If the diagnosis is more than 2 years old you will need a Clinical Confirmation Form and updated referral from your Pediatrician
  • Front and Back of Medicaid Insurance Card
